Special Event Liquor License Application

  1. Select one of the following options and fill in the corresponding information.

  4. (This would be a City liquor licensee (i.e., store establishment) or an IL licensed distributor)

  5. Does licensee’s existing liquor license include an Off-Site Retailer’s Special Use Option (Option VI)? This option allows a liquor license holder of a Class A, AA, B, D or F license to sell alcoholic liquor on any approved City property or right-of-way other than the premises covered by the existing license.

      

       
    • If yes – the special event liquor license fee is $25 for each event; an event may cover multiple dates as long as it is at the same location on all dates, is for the same purpose for all dates, and is submitted under one special event liquor license application for all dates.
    • If no – liquor licensee must add option VI Off-Site Retailer’s Special Use to the existing license:
      - Annual cost: $100 (expiration date is September 30 of each year);
      - Event fee: licensee must also pay the $25 special event liquor license fee for each event; an event may cover multiple dates as long as it is at the same location on all dates, is for the same purpose for all dates, and is submitted under one special event liquor license application for all dates.
